Early detection programme for lung cancer in La Rioja. The first 9 months

Lung cancer (LC) is the leading cause of oncological mortality in Spain. La Rioja is the only community in Spain that has implemented early detection of CP in its service portfolio since May 2023. We present the results of the first 42 patients. Inclusion criteria: patients between 50-75 years old with a history of smoking with a cumulative consumption of more than 20 packs/year who currently smoke or who have stopped smoking in the last 15 years. It was recruited in a single health center through Primary Care consultations, offering a smoking cessation program to all current smokers. Spirometry to all included. 42 patients were included. 31 women (58+-8.1) and 17 men (62.5+-7.6). Tobacco consumption (IPA) was 35.3+-12.8 in women and 36.3+- in men (higher in ex-smokers) 40 valid spirometries with 22 obstruction criteria, 10 men. The diagnosis of COPD was known in 4 men and 2 women. The severity of the obstruction in men was mild in 3, moderate in 6 and severe in 1 (2,3 and 1 in not previously diagnosed). In women, the 10 unknowns corresponded to 7.2 and 1. 28 CT scans have been reported: 3 with nodules larger than 10 mm (under study), 12 emphysema, 5 with coronary atheromatosis and 6 with nodules smaller than 10 mm. An early detection program for CP is feasible from Primary Care, as long as the inclusion criteria are met and it is associated with a smoking cessation program. Greater participation of women. Performing spirometry in PC is feasible, with prior training. Many patients with COPD are detected, especially in women, not suspected clinically. The CT scan detects many alterations, especially emphysema. 18% require additional tests.
