Structured COPD appointment in Primary Care Services

Clinical Research Results Abstract Research Idea Abstract Service Development & Evaluation Abstract To create a structured multidisciplinary appointment (doctor/nurse) in Primary Health Care (PHC) for Chronic Obstructive Pulmonary Disease (COPD) patients to ensure adequate follow-up and health gains. Our goal is: reach at least 75% of our COPD patients; decrease at least 25% of exacerbations and 25% of smokers; control FEV1 in at least 65% of patients; cover at least 60% of patients with Influenza, COVID-19 and Pneumonia vaccination COPD is a common, preventable and treatable condition that is related to smoke, affecting 14.2% of portuguese population over 40 years old. It's responsible for 6% of deaths worldwide, being the third leading death cause. Its diagnosis and classification ncludes lung function (FEV1) from spirometry, symptoms, the impact on daily life and the number of exacerbations. It will determine which treatment and follow-up to be applied in each patient. Smoking cessation and inhaled therapy are the mainstay of treatment. Annual spirometry and vaccination against influenza, COVID-19 and Pneumonia are recommended.Annual COPD appointments in PHC (or home if necessary) with its own registration sheet (attached) and the following management such as: Spirometry Request; Ensuring COPD treatment and correct inhalation technique; Ensuring access to information to obtain social benefits and support networks in all eligible cases; Ensure the orientation of the most complex cases for hospital; Ensuring care in acute illness and therapeutic reconciliation; Addressing smoking cessation, adapted physical exercise, BMI control and measures to prevent exacerbations; Vaccination.The evaluation will be carried out after one year of intervention, comparing the pre and post appointments with greater focus on the scope of the appointment, reduction of exacerbations/patients using emergency services or open appointments, FEV1 control, smoking cessation, therapeutic adherence and vaccination.These measures will reduce risk factors, improve COPD control and increase empowerment and accountability of the patient, with consequent health gains.
